Please Describe the Incident Thoroughly | While traveling north on broadway street in Chelsea I did not see any warning signs about raised manhole covers and there were no traffic cones or barrels attached to the raised black mats circling the manhole covers essentially hiding the fact they were raised and they are meant to be attached to for warn drivers of any obstacles in the road and for safety reasons. Once they were visible to me I slowed down about 15 mph below the speed limit as quick and safely as I could and then swerved to avoid one manhole and straddled the other one to avoid going into opposing traffic. My car was then struck underneath by said raised man hole cover setting off my airbags, cracking my radiator, and front bumper was cracked and falling off, damaging my exhaust, and then my cellphone was thrown from its car phone mount into my windshield shattering my windshield and cell phone. |

Additional detail on location (if needed). | Raise me in all covers were located directly in the middle of the intersection between Eleanor and Broadway. The address is the convenience store right next to it. That’s the most accurate closest address. The additional documents submitted as a photo of the cones that were put onto the manhole covers after I had reported the incident and had the police come. I really wish those had been on there beforehand.. |
