| Please Describe the Incident Thoroughly | See attached email for the description of the issue. In addition to the cut & cap job by A1 pipe, the company also raised the main water line valve to the street on 12/4/2024 (photo attached of completed job). Jorman Mota from the City of Chelsea had confirmed this was necessary after A1 Pipe's recommendation, due to the potential issue of the fire dept not being able to find/access the main water line in the event of a fire, which before the job was covered by debris, as well as being a trip hazard. This was another part of original construction of our building's utility jobs that were not done properly, but approved by the City. In addition to the costs totaled in the attached email, the sewer line issue requires an additional 12 months of pest monitoring of the crawl space area by the existing company, Assurance Pest Solutions, to ensure the solution was sufficient. Those costs are included in the attached excel of $27,345, as detailed. The attached photos from google maps (2024) compared to the current photos of the pavement on Suffolk Street outside of our building show there was no cut in the pavement prior to the job being completed on 12/4/2024 and confirm the cut & cap job was never completed as part of the building's original construction. | Describe the Conditions you choose | No condition applies to our situation, so "construction" was chosen since the issue has to do with the City of Chelsea inspecting a construction project and signing off on a part of the job that was not completed. The date input was the date of the cut & cap project and the time is an estimate, as the job took approximately a half day. |
