| Please Describe the Incident Thoroughly | Construction on corner of Vale St. and Boston St. caused tire puncture with large screw. Unable to be repaired with a simple patch requiring replacement of tire. I drive only 3 miles to and from work every day passing by this construction site. The road is currently in disrepair on this part of the intersection. On the way home this past Friday after passing the diassembled road I noticed a strange sound coming from my tire sounding like a rock was stuck only to discover it was a nail or screw punctured all the way through. Total repair and replacement cost came to : $294.64 |

| Describe the Conditions you choose | There is ongoing construction for the new apartment building being built on this street. The past couple of weeks this portion of the street has been torn up exposing drains and pipes as well as some pretty large bumps and breaks in the cement. There is debris visible as well. |
