| Please Describe the Incident Thoroughly | I was coming down Crescent Ave at 6pm on 5/09/24 and there was a raised Manhole cover and it hit and flatten both tires on the passenger side of my 2018 Ford Ecosport. Both tires were gashed and flatten when I hit the manhole cover |

| Additional detail on location (if needed). | The manhole cover is covered in pavement and raised up like a speed bump and not visible when driving and the tar on the left side is worn away and the hard medal like surface is open and exposed and that is what gashed my tires. It cost me $260.00 to replace both tires and I would like the money back |
