| Please Describe the Incident Thoroughly | on the 30th of January, while driving on highway one within the borders of the city of Chelsea, I hit a pothole just before entering the area of the admirals hill, which lead to the destruction of my car's wheel and a flat tire, as attached, it caused me to suffer financially trying to salvage the damage that happened to my car, as a lawful resident and a tax payer of the state of Massachusetts, I am submitting this form requesting to be reimbursed for the financial burden I had to live through trying to fix my only way of transportation and a big part of my livelihood, specially that the event happened while I was on my way to work. |
