| Please briefly describe your organization/group and its mission or purpose: |
The mission of JWCH is improve the health status and well-being of under served segments of the population of Los Angeles Area through the direct provision or coordination of health care, health education, services and research. The mission is being accomplished through a variety of programs and activities such as: Medical outreach and referrals for medical care, HIV Services and drug treatment; health education; psychosocial assessment and intervention; primary medical care; family planning services; and research.

| Please briefly describe the Organization/Group's connection to LGBTQ Pride |
Our organization is connected with the LGBTQ Pride community and has made a presence in other Pride events in surrounding areas such as Long Beach Pride, DTLA Pride, Downey and Whittier Pride. We have been one of the health care providers who has provided Monkeypox vaccines at the DTLA Pride event last year. In addition, our organization is constantly working in providing the LGBTQ community a safe space in a healthcare setting; and eliminating all barriers in providing preventative services and care.

| Please briefly describe the Organization/Group's connection to West Hollywood |
Our organization has a presence in the surrounding Hollywood area, as we have a clinic in the East Hollywood that provides, Free STD Testing, PrEP/PEP, HIV Services, and hosts a food pantry for HIV positive clients. Our clinic also holds a men's clinic Monday - Thursday from 5:00pm - 8:00pm and we are the only agency who provides free Rapid HIV and Syphillis testing at the North Hollywood bath house. In addition, we provide primary care services to the youth in the Convenant House of California, and are on the campus of Los Angeles City Community College. We want to fortity and continue to form our connections with the West Hollywood community and make our resources and services available to the community.
