| Please Describe the Incident Thoroughly | City of Chelsea - Law Department Att: City Clerks Office Chelsea City Hall, Room #307 500 Broadway Chelsea, MA 02150 Re: Accident near 88 Revere Beach Pkwy, Chelsea, MA 02150 Route 16 West I'm writing to the City of Chelsea regarding my claim for damage to my vehicle in connection with an accident which occurred at above location on Sunday, July 24th 2022 at 9:15pm. The circumstances of the accident are that I was proceeding correctly along Revere Beach Pkwy on the posted speed limit when my vehicle entered a depression in the road due to a large pothole situated in the road. I have enclose copies of photographs to assist you in identifying the exactly accident location. A description of my vehicles damage is as follows : ⦁ Front tire and front wheel damage; ⦁ Rear tire, rear wheel and rear quarter panel damage (tire blowout) ⦁ Front and rear suspension damage; ⦁ Cracked windshield. As a result I have outlaid the following : ⦁ Auto Repair cost (labor + parts) = U$ (480.00 + 750.04) = U$ 1,230.04 ⦁ Body Shop Repair cost = U$ 800.00 ⦁ Tire replacement = U$ 683.05 ⦁ Windshield replacement = U$ 253.30 ⦁ Total = U$ 2,966.39 I therefore look forward to receiving an acknowledgement and a response of this letter on how to receive compensation for these damages and losses.
